private duty Shifts

LPN Private Duty:

Must wear scrubs or client approved clothing to shifts.
Bring time slip and nurses notes to each shift.
Begin each shift note at start of shift, document every 2 hours, and end shift with shift note.
Submit Time slip and Nurses note by the end of each week.
Patient/family must approve time slips

CNA / SITTER Private duty:

Must wear scrubs or client approved clothing to shifts.
Complere notes for each shift and weekly log.
Shift notes should include all care provided during the shift.
Patient/family must approve time slip/log.
Complete CNA shift note daily, including the time that your assisted the patient with that task:
  • Bathing
  • Meal prep and Meal % eaten
  • Toileting / Diaper changes
  • Walking / Ambulating / Turning
  • etc...
Complete weekly time log for your hours worked:
  • Dates worked
  • Day of the week worked
  • Time in and out of each shift
Have patient/family approve your time log.
If time slip or nurses notes are turned in late, your pay check may not be paid on time.
